Join together with Home Hospice Association to ensure no one in your community dies alone or in distress. Collaboration ensures that no member of your community has to leave their “home” to find free, quality hospice care.

We work hand in hand with the community at every step of the way to assist with getting their programs up and running and provide ongoing support as required.   It is our responsibility to build the infrastructure and assist in the marketing and fundraising for both the Chapter and the programs it has chosen to launch. We provide consulting and training to establish the framework required including operational support and guidance, training resources for staff and volunteers, and methods of care and delivery for the hospice program.   

 

Home Hospice Association continues to provide ongoing support, consulting and training to the community as needed once their programs have launched, so that it has the capacity to continue carrying out the programs and to monitor that Home Hospice Association’s care delivery models are constantly being improved and modified to meet the needs of the communities.

Full utilization of the existing knowledge and skills of all community members in collaboration with established best practices to develop a cohesive and inclusive standard of care that keeps the needs of the dying person (and those who love them) at the centre of care.
